Initial Comments:
An off site follow up survey conducted on 5/10/2022, Homecentris Healthcare, corrected the deficiencies and was found to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries. The deficiencies were cited as the result of a relicensure survey conducted on 3/17/22.
